Referrals are preferred to ensure proper triage of scheduling patients and are accepted from clinicians, primary care providers, case managers, clinics, and hospitals.

Patients can be referred to Sage Mental Health by faxing a referral

with pertinent office notes and demographic information

We currently have a waiting list for new patients. Each referral and intake is carefully reviewed so we can triage and schedule patients appropriately. All intake paperwork must be completed before scheduling, and a referral from your primary care provider is preferred. Please call or email us for more information.

Documents to include with all referrals:

The patient’s last office note and other pertinent documentation
A Release of Information,
Patient Demographic Information
