Crystal LeBoeuf MSN, ARNP, PMHNP-BC
Crystal is a Board-Certified Psychiatric Nurse Practitioner who provides diagnostic evaluations, medication management, and treatment planning for children, adolescents, and adults. She treats a wide range of mental health conditions including depression, anxiety, PTSD, ADHD, bipolar disorder, and insomnia. She is also the founder and owner of Sage Mental Health.
Crystal believes mental health treatment involves more than just medication. She uses a collaborative, individualized approach that may include medication, lifestyle changes, supplements, therapy, and coordination with other providers. She values patient autonomy and strives to ensure each patient feels heard, respected, and actively involved in their treatment.
Crystal has over 16 years of nursing experience with a background in emergency medicine, primary care, and inpatient care.

Kara Heath MSN, ARNP, PMHNP-BC
Kara Heath is a board-certified Psychiatric–Mental Health Nurse Practitioner with 16 years of nursing experience. Her clinical background includes work in both correctional healthcare and the emergency department, where she developed a strong foundation in managing complex and high-acuity patient needs.
Kara pursued advanced training as a PMHNP with a commitment to improving access to mental health care in the community where she grew up. She believes effective psychiatric care is built on collaboration and patient autonomy, ensuring individuals are active participants in their treatment decisions. Outside of her professional work, Kara enjoys reading, traveling, and spending time near the ocean.

Breck has embarked on this path with a mission: to dismantle the stigma surrounding mental illness and substance use, ensuring access to compassionate, quality care for all. This commitment isn't just professional; it's personal. It’s the core reason Breck became a PMHNP — to make a tangible difference outside the constraints of traditional employment politics, which often hinder genuine patient-centered care.
With expertise in treating mood disorders, depression, bipolar, anxiety disorders, psychotic disorders such as schizophrenia, schizoaffective disorder, and supporting those in recovery from substance use, Breck offers a sanctuary for healing.

Sage joined the practice in March 2025 after being rescued through For the Love of Dogs Vermont, and she has taken her role as office greeter very seriously ever since.
Born on November 24, 2024, Sage is a mix of Catahoula Leopard Dog, German Shepherd, and Labrador Retriever — which essentially means she is smart, loyal, energetic, and always aware of where the snacks are.
Sage’s primary responsibilities include greeting patients at the door, offering canine kisses, providing tail wags, and occasionally reminding everyone to take breaks and go outside. She believes strongly in the therapeutic benefits of dogs, especially on difficult days.
When she is not working, Sage enjoys playing with kids, chewing bones, carrying her toys around the house, and preparing for her next day at the office, which is still her favorite place to be.
